Bookkeeping for medical practices in Columbus, OH
Payer mix, contractual adjustments and refunds handled the way a practice actually gets paid — not the way a retail shop does.
A medical practice almost never collects what it bills. Between contractual adjustments, patient responsibility, refunds back to payers and the percentage a billing company takes off the top, the money that lands in the account has been through four subtractions — and books that ignore them produce a profit figure nobody believes.
We record each subtraction where it belongs: adjustments against gross charges, refunds against income rather than as an expense, billing fees as the cost they are. Patient credits stay a liability until they are earned or returned.
What you get back is a close that reconciles — deposits to remittances, remittances to charges — and a payer-mix view that shows which contracts are worth keeping.
What your books actually need
- Contractual adjustments taken against gross charges, so billed, allowed and collected all reconcile
- Payer mix reported by insurance versus patient responsibility, month over month
- Refunds to payers and patients recorded against income, never as an expense
- Billing-company and clearinghouse fees shown as a cost of collecting, not netted into deposits
- Locum and contracted provider payments tracked for clean 1099s at year end
- Patient prepayments and credit balances carried as a liability until earned or refunded
What it costs
Or $124/month billed month to month. One flat fee. No setup fee.

Do you work with my CPA?
Yes. Most practices keep the accountant they already have. We keep the books through the year and hand over a clean, reconciled file at year end — so they file from it rather than rebuilding it in March.
Do you handle patient records?
No, and we ask you not to send them. We work from bank, card and merchant feeds and the summaries your practice software already produces — dollars, not charts. We are not a HIPAA business associate, so please keep patient-identifying documents out of anything you upload to us.
We already have a billing company. Do you replace them?
No. They chase the claims; we account for what comes back. The two jobs are different, and a practice generally needs both — what we add is a reconciliation between what was billed, what was allowed and what actually reached the bank.
We're behind. How far back can you go?
As far back as you need. Catch-up work is quoted separately from the monthly fee, once we've seen how many months are open and how many accounts are involved — it is a one-off project, and pricing it as one keeps your monthly number honest.
We have several providers. Can you report per provider?
Yes. Income is tracked to the provider who earned it and set against what that provider costs, so a compensation conversation starts from a figure rather than an impression.
